Tri-County Office on Aging is seeking a Registered Nurse to join a Supports Coordinator team in Lansing, MI. The role focuses on developing person-centered care plans, assessing health and functional needs, and coordinating services for elderly and disabled participants.

The position requires a Michigan RN license, 1 year of nursing experience, and travel across Clinton, Eaton, and Ingham Counties. Office hours are Monday–Friday, with on-call duties as scheduled.

Qualificações

• Current Michigan Registered Nurse (RN) license in good standing.
• One year of nursing experience.
• Valid Michigan driver’s license with acceptable driving record.
• Ability to travel throughout Clinton, Eaton, and Ingham Counties.

Responsabilidades

• Assist participants in securing Medicaid eligibility for Waiver services and monitor ongoing eligibility.
• Develop person-centered care plans with each participant.
• Conduct assessments of health, functional status, support systems, and environment as part of a professional team.
