Central City Health in Detroit, MI is hiring a Nurse Care Coordinator to oversee an adult panel, coordinate care across primary, behavioral health, and housing services, and perform medication reconciliation. The role requires strong triage skills and experience in ambulatory settings.

You will collaborate with the Nursing Director and a multidisciplinary team to enhance continuity of care, implement proactive outreach, and ensure timely interventions for complex cases across service lines.

Qualifications

• Active, unrestricted Michigan RN licensure.
• BLS certification required.
• Strong clinical triage judgment including medication reconciliation.

Responsibilities

• Own a panel of CCV-eligible adult members at the site continuously.
• Monitor the EHR for qualifying events between visits.
• Perform medication reconciliation and follow-up with Pharmacist consults.
