Job summary

Prisma Health’s Tuomey Hospital in Sumter, SC seeks a patient-focused Nurse Case Manager to coordinate transitions from acute to post-acute care and support care-plan alignment. You will collaborate with physicians, nurses, and payors to optimize quality and resource use.

Requires a BSN and at least 3 years acute care nursing experience; ACM/CCM preferred. ADN or Nursing Diploma acceptable with BSN pursuit within four years. Excellent patient advocacy and communication skills are essential.

Qualifications

• Bachelor's degree in Nursing required.
• Three years acute care nursing experience; one year acute care case management preferred.
• In lieu of BSN, nursing diploma or ADN may be considered with pursuing BSN/MSN within four years.

Responsibilities

• Partner with attending providers during hospitalization to promote efficient use of resources and ensure quality, cost-effective care.
• Identify social determinants of health and coordinate post-acute plans with ambulatory care management.
• Advocate for patient and family empowerment and navigate value-based care to align with goals.
