Job summary

In collaboration with physicians, you will lead the multidisciplinary team to ensure a smooth, patient-centered care plan.

Qualifications

• Bachelor's degree in Nursing.
• Three (3) years acute care nursing experience; one (1) year case management preferred.
• Holds a current RN license recognized by the state (or multi-state compact).

Responsibilities

• Partners with attending providers during hospitalization to promote efficient utilization of clinical resources.
• Identifies social determinants of health and coordinates post-acute plans with ambulatory care management.
• Navigates regulatory and insurance requirements and documents delays or obstacles to care.
