Qualifications

• One (1) year acute care case management experience preferred.

Responsibilities

• Coordinate care transitions from acute to post-acute settings.
• Collaborate with physicians and multidisciplinary teams to ensure quality, cost-effective care.
• Identify social determinants of health and coordinate post-acute plans with ambulatory teams.
• Ensure documentation for the care team and communication with payors.
• Promote adherence to regulatory requirements and utilization reviews.
