Job summary

Prisma Health is seeking an experienced nurse to join the BM Hospital Care Management team as an integral care partner focused on patient engagement, transitions, and longitudinal care plan alignment. You will work with physicians and a multidisciplinary team to promote efficient, high-quality, cost-effective care for patients in the Blount Memorial Hospital system.

The role requires a BSN or higher, three years of acute care nursing, and an active RN license; CCM/ACM credentials are preferred.

Qualifications

• Bachelor’s degree in Nursing.
• Three years acute care nursing experience; one year acute care case management preferred.
• Active RN license in state of practice.

Responsibilities

• Coordinate patient care transitions from acute to ambulatory settings.
• Collaborate with physicians and multidisciplinary team to ensure quality, cost-effective care.
• Identify social determinants of health affecting patient outcomes.
• Advise on utilization of clinical resources and care plans.
