Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN (CC RN) for the Day shift. A minimum of 1 year of acute care experience is required to assess transitional care needs and coordinate care across the continuum while engaging with patients and families to ensure care needs are met.

The role emphasizes care progression, discharge planning, and efficient resource utilization, in collaboration with the care team.

Qualifications

• Registered nurse with acute care experience; required minimum.
• Strong written and verbal communication skills; able to interact with patients, families, and care teams.
• Ability to collaborate across disciplines and coordinate care transitions.

Responsibilities

• Assess patients' chronic disease management needs and psychosocial risk factors.
• Coordinate with PAS, financial counselor and UM nurse to assess insurance needs.
• Collaborate with patient, family, physicians, and care team to support discharge plans.
• Meet with physicians and care team to review patient management and discharge timing.
• Facilitate communication about patient progression to ensure timely care.
• Refer suitable cases for social work intervention and support post-acute referrals.
