Southwell, Inc. is seeking an experienced Case Manager (RN) to coordinate care for discharge planning and complex patients across the continuum. You will work with physicians, nursing staff, social workers, and community resources to ensure quality clinical and cost outcomes.

You will assess needs within 24–72 hours, review plans of care, and facilitate timely hospital-to-outpatient transitions while maintaining HIPAA compliance and quality data collection.

Qualifikationen

• Associate's Degree in Nursing required and active RN license.
• Minimum of three years in acute care hospital settings or discharge planning.
• Ability to coordinate care and communicate with physicians, care teams and patients.

Aufgaben

• Screen and assess patients for discharge planning needs and resource management.
• Continuously review and reassess assigned patients and plans of care.
• Intervene to manage resource utilization and plan of care.
• Evaluate trends that could impact hospitalization length or readmission risk.
• Communicate plan of care and approvals with physicians and care coordinators.
• Provide resources on insurance payments, home care services, and community resources.
• Collaborate with inpatient care coordinators to ensure safe transitions to outpatient settings.
• Assist in educational programs for staff and community on case management activities.
• Identify opportunities to improve resource utilization and reduce costs.
• Facilitate access to test results and timely consultations.
• Assist transfers to other facilities as needed.
• Document patient care in the medical record and collect outcome indicators.
