Job summary

Qualifications

• Licensure as Registered Nurse in RI or compact licensure per state rules.
• Bachelor's degree required; Certification in Case Management preferred.
• Three years of clinical experience with case management or discharge planning preferred.

Responsibilities

• Partner with medical staff to facilitate the plan of care across the continuum.
• Identify high-risk patients and target interventions within one business day of admission.
• Participate in daily care rounds to evaluate and implement discharge planning.
• Develop initial discharge plan and review with patient, family, and team.
• Reassess discharge plan daily during collaborative rounds.
• Coordinate length of stay and inform care team of insurance qualifiers.
• Communicate discharge plan to patients and families; ensure safe handovers.
