Job summary

Texas Children's Hospital is seeking a Care Coordinator to partner with physicians and an interdisciplinary team to assess, plan, implement, and monitor individual health needs. The role focuses on comprehensive case management, optimizing outcomes, and ensuring cost-effective care across the continuum.

The candidate will guide resource allocation, support families, and ensure accurate medical documentation while maintaining professional communication with care teams.

Qualifications

• Bachelor's degree in nursing and RN licensure required.

Responsibilities

• Assess, develop, implement and monitor a comprehensive plan of care through an interdisciplinary team process with the patient/family.
• Review clinical documentation and collaborate with medical and nursing staff to ensure appropriate level of care and patient placement.
• Identify problems and discuss with the team to mitigate barriers to implementing the plan of care.
• Coordinate timely transition of information to unit Care Coordinators to support revenue cycle processes.
• Complete visits with patients across the continuum of care as indicated.
• Participate in orientation and training of new department members.
• Identify and monitor quality improvement opportunities along the continuum of care.
• Identify and report safety and quality issues to Quality Management.
