Texas Children’s Hospital in Houston seeks a dedicated Care Coordinator to assess, plan, implement, monitor and evaluate patient care across the continuum. You will partner with physicians to allocate resources, support families, and optimize self-care and consumer rights.

The role requires RN licensure, a nursing degree, and at least 3 years in acute care, with preferred experience in pediatric care, home care or managed care.

Qualificações

• Bachelor’s degree in nursing or an associate's in nursing with enrollment in a BSN program.
• Current RN licensure by the Texas Board of Nursing or Nursing licensure compact.
• BLS from AHA.
• 3 years of nursing experience in an acute care setting.
• Experience in community health, complex pediatric patient care, home care, case management, managed care, or utilization review highly preferred.

Responsabilidades

• Assesses and develops comprehensive plans of care with an interdisciplinary team.
• Reviews clinical documentation for appropriate level of care and patient placement.
• Identifies problems and facilitates interventions to overcome barriers to care.
• Serves as content expert on level of care and coordinates resource use.
• Screens patient for level of care and ensures medical record reflects requested level.
• Coordinates timely information transfer to the care unit to support revenue cycle.
• Completes patient visits across the continuum of care as indicated.
• Participates in orientation and training of new department members.
• Monitors quality improvement opportunities along the continuum of care.
• Reports quality, safety, and service issues to Quality Management.
