Job summary

Texas Children’s Hospital in Houston is seeking a Care Coordinator to assess, plan, implement, monitor and evaluate services to meet individual health needs. You will provide comprehensive case management, coordinating care across the continuum with physicians to optimize outcomes and resource use.

The role requires RN licensure in Texas or compact, a nursing degree, BLS certification, and 3 years of acute care experience; pediatric and home care exposure is highly valued.

Qualifications

• Bachelor’s degree in nursing or an associate’s in nursing with BSN enrollment.
• Current RN Licensure by the Texas Board of Nurses or Nursing licensure compact.
• BLS from AHA.
• 3 years of nursing experience in an acute care setting.
• Experience in community health, complex pediatric care, home care, case management, managed care, or utilization review preferred.

Responsibilities

• Assess, develop, implement and monitor a comprehensive plan of care with an interdisciplinary team.
• Review clinical documentation and coordinate patient placement and level of care.
• Identify problems and coordinate interventions with the multi-disciplinary team.
• Serve as care coordination content expert and maintain open communication.
• Screen patients for level of care and ensure accurate medical records reflection.
• Coordinate timely transition of information to unit Care Coordinator for revenue cycle.
• Complete visits across the continuum and participate in orientation and training.
• Identify and monitor quality improvement opportunities; report safety and quality issues.
