Texas Children's Hospital in Houston seeks a Care Coordinator to provide comprehensive case management for pediatric patients. The role involves coordinating care across settings, assessing needs, and working with interdisciplinary teams to optimize outcomes.

The ideal candidate holds an RN license in Texas, has 3+ years acute care experience, and may have experience in community health or utilization review. This is a full-time, on-site position within a leading pediatric health system.

Qualifications

• Bachelor’s degree in nursing or an associate’s in nursing with enrollment in a BSN program.
• Current RN Licensure by the Texas Board of Nurses or Nursing licensure compact.
• BLS from AHA.
• 3 years of nursing experience in an acute care setting.
• Experience in community health, complex pediatric patient care, home care, case management, managed care, or utilization review highly preferred.

Responsabilités

• Assesses, develops, implements and monitors a comprehensive plan of care through an interdisciplinary team process with the patient/family.
• Reviews clinical documentation and collaborates with medical and nursing staff for appropriate level of care and patient placement.
• Identify problems and discuss with the team to mitigate barriers to plan of care implementation.
• Serve as content expert on level of care and maintain open communication with the care team.
• Screen patient for level of care and ensure medical record reflects requested care level.
• Coordinate timely transition of information to unit Care Coordinator for revenue cycle support.
• Complete visits with patient across the continuum of care as indicated.
• Participate in orientation and training of new department members.
• Identify and monitor quality improvement opportunities along the continuum of care.
• Identify and report quality, safety and service issues to Quality Management.
