Job summary

Westchester Institute for Human Development (WIHD) seeks an experienced, skilled Care Coordinator to join the Chronic Care Management (CCM) program. This fast-paced, patient-centered role requires strong communication, organization, and the ability to manage multiple patients daily, coordinating with providers, caregivers, and patients to close care gaps.

You will conduct structured CCM outreach calls, review charts, coordinate follow-up, and document all interventions in the EMR.

Qualifications

• Active licensure, registration, or certification in a related healthcare field is required and must be maintained in good standing.
• 2+ years of experience in healthcare or administrative clinical settings.
• Experience with medical scheduling, referrals, or patient coordination.
• Working knowledge of electronic medical records (EMR) systems.
• Excellent communication skills (written, phone, email, and patient-facing interaction).
• Ability to work independently while following structured workflows.
• Strong clinical judgment, chart review skills, and EMR proficiency.
• Ability to manage high-volume patient panels independently.

Responsibilities

• Conduct CCM outreach calls to assigned patients.
• Review patient charts prior to calls to identify care gaps and needs.
• Follow up on appointments, referrals, labs, imaging, and provider recommendations.
• Coordinate with providers, specialists, and internal care teams.
• Ensure continuity of care following hospital or emergency visits.
• Accurately document CCM encounters in the EMR with patient-specific details and actions taken.
• Track follow-up items until resolution and maintain CCM compliance.
