Cleveland Clinic Akron General Hospital is seeking an RN Care Coordinator in Addiction Recovery to support patients who come to the Emergency Department seeking help with substance use recovery. You will assess needs, educate on options, and coordinate referrals to community resources, serving as a liaison with behavioral health providers to ensure appropriate care and continuity.

Schedule features two 12-hour shifts totaling 24 hours per week, with rare weekend coverage and no holiday/on-call

Formación

• Graduate from accredited school of Professional Nursing and be licensed as an RN.
• Current BLS certification (AHA or ARC).
• 3–5 years of nursing experience, preferably Oncology or Ambulatory care.
• BSN is preferred.

Responsabilidades

• Assess patient needs and outline care coordination requirements for the specialty team.
• Educate patients on treatment options and coordinate referrals to addiction recovery resources.
• Coordinate transitions of care and facilitate follow-up to support engagement in recovery.
• Serve as liaison with behavioral health providers and recovery organizations to connect patients with appropriate care.
