Job summary

Gosnold, Inc. in Plymouth, MA seeks an ED Navigator (8 hours, 7a-3p) to assist patients in the hospital ER with substance use disorders and to connect them with addiction-specific services.

The role requires a counseling-related bachelor's degree and experience in behavioral health, with coaching certification training encouraged. Responsibilities include screening, referral, and follow-up to support recovery within a patient-centered care framework.

Qualifications

• Bachelor's degree in counseling or related field and/or one year of experience.
• Two years of experience with substance use disorder patients in behavioral health.
• Ability to engage with patients in crisis with hope, respect and empowerment.
• Willingness to undergo recognized coaching certification.

Responsibilities

• Engage with patients in the ER in a non-judgmental manner and refer to addiction-specific services.
• Conduct screening assessments to determine suitability for treatment referrals.
• Provide recovery coaching, role modeling, and mentoring to maximize referral placement.
• Coordinate transfers to treatment settings and arrange transportation when needed.
• Develop referral relationships with detox, rehab, and community resources across a wide area.
• Perform follow-up phone contacts with patients and families who refuse treatment referrals.
