Job summary

Ascension is seeking a Registered Nurse in Florida to coordinate care across the continuum, integrating patient and family preferences and values. You will monitor patient care through assessments, evaluations and/or patient records, and advocate for resources to remove barriers to care.

You will maintain ongoing dialogue with supervisors and health providers to ensure effective implementation of the health plan, and serve as a resource for proper medical record documentation and post-discharge

Qualifications

• RN license in the state of Florida is required.
• BLS Provider certification preferred (American Heart Association or American Red Cross).
• Diploma from an accredited nursing program or current professional licensure at time of hire.

Responsibilities

• Create plan for care across the continuum, integrating patient/family preferences and values.
• Monitor patient care through assessments, evaluations and/or patient records.
• Advocate for resources and removal of barriers.
