Baptist Health Princeton Hospital seeks an RN to collaborate with the clinical team in identifying patients who will benefit from care coordination services. The role involves assessing risks, coordinating care, and ensuring effective discharge planning across settings.

You will educate patients and families, advocate for patient needs, and support timely access to services, while maintaining safety and compliance with policies.

Qualifications

• Requires a BSN degree.
• Active RN license in Alabama.
• BLS/Healthcare Provider certification.
• Minimum 3 years direct clinical experience.

Responsibilities

• Lead continuity of care across inpatient, emergency, and ambulatory settings.
• Collaborate with care team to engage patient/family and ensure discharge planning.
• Monitor discharge plans daily and adjust as patient conditions change.
• Educate patients and families on chronic illness and self-management strategies.
• Coordinate access to prescriptions, DME, and other services.
• Support patient safety and compliance with policies.
