Job summary

BMCHS is seeking an RN Care Manager for the Emergency Department. The role supports care coordination across the continuum, assessing needs, planning interventions, and monitoring outcomes to promote quality and cost-effectiveness.

The position requires an RN license in MA, a nursing degree, and 3–5 years of clinical experience with case management. CCM is preferred; ED experience is a plus. Weekend evenings and one weekday evening shift are required.

Qualifications

• Nursing degree: Diploma, ASN or BSN preferred, BSN within 4 years.
• Licensed to practice as an RN in Massachusetts by start date.
• CCM or related certification preferred within 24 months of hire.
• 3–5 years of diversified clinical experience including 1 year case management.

Responsibilities

• Assess, plan, implement, monitor and evaluate options and services for health needs.
• Identify appropriate providers and facilities across the continuum of services.
• Ensure resources are used timely and cost-effectively for optimum value.
• Support the care management process and department during high volume.
