Job summary

Qualifications

• Nursing degree or diploma with ASN/BSN preferred and license to practice in MA by start date.
• 3–5 years of diversified clinical experience; 1 year in case management required.
• Knowledge of managed care, health plan contracts, and clinical terminology.

Responsibilities

• Assess, plan, implement, monitor and evaluate patient care options across the continuum.
• Identify appropriate providers and facilities to ensure timely, cost‑effective care.
• Support the care management process and handle increased volume in ED settings.
• Collaborate with multidisciplinary team to promote quality outcomes.
