Job summary

Qualifications

• Active and unrestricted RN licensure (PA or Compact) as required by state of residence
• 3+ years of nursing experience
• 2+ years of case management, discharge planning or home health coordination experience

Responsibilities

• Engage with members telephonically for 50-75% of the day to coordinate care
• Develop individualized care plans with member participation
• Provide disease management education and support to achieve health goals
• Coordinate with interdisciplinary care team to ensure appropriate involvement
• Document care management activities in the EHR
• Identify community resources and health plan benefits for members
