Prominence Health is seeking an RN In-Clinic Care Manager to provide care management for medically and socially complex members under the Care Management Clinical Program. The role emphasizes patient-centered, culturally sensitive care coordination and adherence to CMSA Standards of Practice for Case Management.

Qualifications

• Associate or Bachelor’s Degree in Nursing required.
• Active, unrestricted RN license in the State of practice (Nevada).
• CCM/CMGT-BC/ACM-RN/CMCN certifications preferred.
• Minimum of three (3) years in clinical nursing; three (3) years in Case Management/Transition of Care preferred.
• Experience with Medicare/Medicaid populations and processes preferred.
• Strong clinical triage skills and knowledge of CMS standards.

Responsibilities

• Provide care management services for medically and/or socially complex members.
• Develop comprehensive, individualized care plans and coach for adherence.
• Assess member needs and promote self-care and advance care planning.
• Sustain partnerships with physicians, care coordinators, and community resources.
• Support SNP CM program implementation and ensure compliance with CMS/NCQA.
