Job summary

Manchester Memorial Hospital, Inc. is seeking a Clinical Nurse Educator to bridge care among providers, payers, and patients.

The role emphasizes discharge planning, care coordination, and interdisciplinary collaboration within the Hartford HealthCare network. Ideal candidates will have RN licensure in Connecticut, a BSN or higher, and at least five years of medical-surgical nursing with case management experience.

Qualifications

• Graduate of an accredited school of Nursing.
• Baccalaureate degree in an applicable field required.
• Current license as a Registered Nurse in the State of Connecticut.
• Professional certification in case management or clinical specialty preferred.
• Five (5) years of nursing experience in a medical-surgical acute care setting required.
• Experience in case management, home care, and utilization review preferred.
• Good analytical, organizational and interpersonal skills; ability to communicate effectively in English; basic PC proficiency.

Responsibilities

• Monitors admissions for appropriateness of observation services or inpatient hospitalization per InterQual criteria.
• Identifies patients with discharge planning needs and coordinates necessary services in home or nursing facility settings.
• Coordinates patient care using nursing judgment and interdisciplinary team collaboration.
• Communicates patient’s clinical condition and care needs to appropriate parties.
• Promotes nursing profession and professional practice; may provide coverage at both hospitals as needed.
