Job summary

McLaren Health Care in Michigan seeks a full-time Care Management RN to ensure proactive coordination and timely transition of patients to appropriate levels of care. You will work with physicians, nursing, social work, and other team members to optimise LOS, resource use, and readmission prevention.

Qualifications include an RN license, a Bachelor’s degree in nursing, and at least three years of acute hospital experience. BLS certification and CCM/ACM are preferred.

• Bachelor’s degree in nursing or higher is expected.
• Three years of acute hospital care experience required.

Responsibilities

• Coordinate care and transitions for patients within the care continuum.
• Collaborate with physicians, nursing, social work and case management teams.
• Identify and address barriers to timely discharge and safe transitions.
• Review patient needs and resources to plan appropriate level of care.
• Coordinate referrals to social services and post-acute care as per SOP.
• Identify risks of readmission and implement interventions.
• Assist with discharge planning coordination and referrals.
