Job summary

Qualifications

• RN license required in Michigan.
• 3+ years of clinical nursing experience with chronically ill patients.
• Experience in care coordination or population health is preferred.

Responsibilities

• Provides telephonic and face-to-face comprehensive assessment and care management services as part of an interdisciplinary team.
• Uses risk assessment tools to target high-risk populations and develops individualized care plans.
• Coordinates care with PCPs, specialists, and community resources to prevent hospitalizations and emergency visits.
