Job summary

McLaren Health Care in Michigan is seeking a Registered Nurse for Care Management to coordinate patient transitions across the continuum. This full-time role supports discharge planning and works with physicians, nurses, social workers and other teams to optimize LOS and resource use.

The position requires an RN license and a BSN, with at least 3 years of acute hospital care experience; daily hours are 0800–1600, with a standard full-time schedule.

Qualifications

• RN licensure in the state of Michigan (or compact state).
• Bachelor’s degree in nursing from an accredited program.
• Three years of acute hospital care experience.

Responsibilities

• Perform care coordination assessments within 24 hours of admission.
• Coordinate discharge planning and transition to next level of care.
• Collaborate with social workers and multidisciplinary teams to ensure safe transitions.
• Identify barriers to discharge and facilitate timely planning.
