Job summary

McLaren Health Care is seeking a Registered Nurse Care Manager – Case Management to coordinate patient care and transitions across the continuum in a hospital setting.

You will work with physicians, nursing staff, social workers, and other disciplines to achieve optimal length of stay, cost-efficient resource use, and prevent readmissions. A BSN and RN license are required; three years in acute care are expected, with CCM/ACM and BLS preferred.

Qualifications

• RN licensure is required and must be active.
• Bachelor’s degree in nursing; advanced degree may be preferred.
• Minimum three years of acute hospital care experience.
• Certification in Case Management (ACM/CCM) preferred.

Responsibilities

• Performs care coordination assessments within 24 hours of admission and for readmission and transition planning.
• Collaborates with social workers and other disciplines to ensure safe, timely transition to the next level of care.
• Assesses patient/family needs to reduce barriers and formulate discharge plans.
