Qualifications

• Minimum of three years acute hospital care experience.

Responsibilities

• Performs care coordination assessments for initial patient admission and discharge planning.
• Collaborates with social work, physicians and other disciplines to ensure safe, timely transition to next level of care.
• Identifies patient/family barriers and develops discharge plans to reduce LOS barriers.
