Job summary

McLaren Health Care in Michigan seeks a Care Coordination Nurse to oversee timely transitions of patients to appropriate levels of care, aiming to reduce LOS and readmissions. You will work with physicians, nursing staff, social workers and home care teams to coordinate dispositions across the continuum.

Responsibilities include initial assessments within 24 hours of admission, discharge planning, and ensuring accurate LOC orders while collaborating with utilization management and Cerner-linked

Qualifications

• State licensure as a Registered Nurse (RN).
• Bachelor’s degree in nursing; degree to be obtained within five years of accepting position.
• Three years of acute hospital care experience.
• American Case Management Certification (ACM) or CCM certification when eligible; ongoing education requirements.
• Preferred: utilization management/case management, critical care, or patient outcomes/quality management.
• Preferred: BLS certification for Healthcare Provider.

Responsibilities

• Performs care coordination assessments within 24 hrs of admission.
• Collaborates with social workers and other disciplines for safe, timely transitions to the next level of care.
• Assesses patient/family needs to reduce barriers and formulate discharge plans.
• Identifies unsigned level of care (LOC) orders; communicates with UM nurse and providers.
• Reviews DRG/LOS in Cerner to plan discharge and identify the point of contact.
