Job summary

Bon Secours Mercy Health is seeking a skilled RN Care Manager for Maryview Medical Center to coordinate patient care across the continuum, ensuring safe transitions and effective use of resources. The role emphasizes collaboration with physicians, nurses, social workers and other team members to develop and implement individualized plans of care.

The position requires active case management, documentation discipline, and a strong focus on quality outcomes.

Qualifications

• BLS Basic Life Support – American Heart Association (required)
• RN License in the state in which they are working or covered by compact (required)
• Accredited Case Manager Certification (ACM) from ACM or CCM from CCM Certification or ANCC Nursing Case Management board certification (preferred)

Responsibilities

• Identifies and prioritizes patients in need of care management services and plans with the care team to maximize outcomes.
• Completes documentation in the patient record and ensures timely handovers during care transitions.
• Ensures and reflects the plan of care to address post-hospital needs and resources with patient involvement.
• Supports discharge planning and utilization management per CMS guidelines and practices.
• Participates in department clinical outcome projects and process improvements.
