Job summary

Bon Secours seeks a Social Worker - Care Manager for Maryview Medical Center to provide interventions and discharge planning, ensuring smooth transitions across care settings. The role includes supervision of peers and students, with emphasis on accessing hospital, community, and governmental resources.

Responsibilities include care coordination, documentation, and collaboration with the care team to optimize patient outcomes, while complying with CMS requirements and promoting timely discharge

Qualifications

• Bachelor of Social Work required.
• Master’s degree in social work or healthcare field preferred.
• Licensed Social Worker in VA or eligible for VA licensure preferred.
• BLS Basic Life Support required.

Responsibilities

• Identify and prioritize patients in need of social services using biopsychosocial assessment.
• Plan with patient, caregivers and healthcare team to maximize outcomes.
• Document care plans clearly and timely in patient records.
• Coordinate post-hospital care and community resources as needed.
• Support discharge planning and care transitions with interdisciplinary rounds.
• Collaborate with care team to remove barriers to timely testing and treatment.
• Provide supervision for social workers and students as appropriate.
