ArchWell Health is seeking a Nurse Care Manager to coordinate and evaluate care for patients with acute and chronic conditions across the care continuum. You will develop care plans with the care team and execute them to meet objectives, standards, and quality outcomes.

You will work closely with clinicians, case managers, and families to ensure safe discharge planning, appropriate referrals, and ongoing patient engagement in a value-based care model.

Qualifikationen

• Asscociate degree in Nursing required.
• BSN preferred or RN with related bachelor’s degree.
• Active RN license in state of employment.
• Willingness to obtain license in other ArchWell locations.
