The Nurse Care Manager coordinates and evaluates management of patients with acute and chronic conditions across the care continuum, guiding care from our centers into home and post-acute settings.

Formación

• Active RN license in the state of employment.
• Minimum 2 years’ clinical work experience.
• Experience in utilization review, discharge planning or case management preferred.

Responsabilidades

• Establish trusting relationships with patients, families, and caregivers.
• Collaborate with clinical staff to achieve patient goals.
• Identify high-risk/complex patients and develop comprehensive care plans.
• Coordinate transitions of care from clinics to home or post-acute settings.
• Provide patient education and care coordination to support health goals.
