Qualifications

• Associate degree in Nursing required.
• BSN preferred or RN with bachelor's degree in related field.
• Active RN license in state of employment required; willingness to obtain licenses in other states.

Responsibilities

• Establish trusting relationships with patients, families, and caregivers.
• Collaborate with clinical staff to achieve patient goals.
• Coordinate transitions of care and discharge planning.
• Develop comprehensive, individualized care plans based on identified needs.
• Provide patient education and care coordination to help achieve health goals.
• Communicate with hospital case managers, PTs, social workers, patients, and families to facilitate safe discharge plans.
• Monitor and evaluate progress toward patient goals.
