Job summary

The Greater Manchester Region of Hartford HealthCare seeks a Clinical Nurse Educator/ Care Manager to coordinate transitions of care, monitor admissions, and collaborate with physicians and community partners. The role emphasizes evidence-based discharge planning, patient education, and cross-team communication.

Ideal candidates hold a Connecticut RN license, a bachelor’s degree, and 5+ years of medical-surgical nursing with case management exposure.

Qualifications

• Graduate of an accredited school of Nursing.
• Bachelor’s degree in an applicable field required.
• Must have a current license as a Registered Nurse in the State of Connecticut.
• Professional certification in case management or clinical specialty is preferred.
• At least five (5) years of nursing experience in a medical surgical acute care setting; additional experience in case management, home care, and utilization review/management is preferred.

Responsibilities

• Coordinate patient care by applying nursing judgment and collaborating with interdisciplinary team.
• Monitor admissions for appropriateness of observation services or inpatient hospitalization per InterQual criteria.
• Identify discharge planning needs and arrange services in home or nursing facility settings.
• Communicate patient condition and care needs to providers, payers, and families; ensure age-appropriate care as patients grow.
