Qualifications

Responsibilities

• Coordinate patient care across the care continuum using nursing judgment.
• Monitor admissions for appropriateness per InterQual criteria.
• Coordinate discharge planning and home/nursing facility services.
• Collaborate with interdisciplinary team and report to the Care Management Manager.
