Job summary

Centene is seeking a Care Manager (RN) to develop and facilitate complex care management for members with physical needs, delivering high-quality, cost-effective outcomes. This remote position suits those with OB, NICU, and postpartum care experience.

The role involves evaluating member needs, planning care, coordinating with providers, and ongoing follow-up, while educating families and linking to local resources for timely services.

Qualifications

• RN license in state or compact with nursing degree.
• 2-4 years of related care management experience.
• Ability to develop and update care plans and educate patients.

Responsibilities

• Evaluate the needs of the member and recommend care plans for the best outcomes.
• Develop ongoing care plans and collaborate with providers to address unmet needs.
• Identify problems/barriers to care and provide appropriate interventions.
• Coordinate between the member and care provider team for person-centered care.
• Provide ongoing follow-up and monitoring of member status and care plan goals.
• Offer resource support for local services based on assessments.
• Facilitate care management and ensure timely access to needed services.
• Perform telephonic, digital, and home outreach to assess member needs.
• Collect, document, and maintain member information for compliance.
• Educate members and their families on disease processes and healthcare options.
