Job summary

Intermountain Healthcare Inc. seeks an RN Hospital Care Manager I to deliver comprehensive care management across inpatient and outpatient settings. You will assess patients, develop and adjust care plans, and guide self-management with motivational interviewing to improve outcomes.

Collaboration with care teams and stakeholders is essential. Applicants should hold an RN license in their state and a BSN, with BSN required within four years.

Qualifications

• RN license in state of practice.
• BSN within four years of hire/promotion unless an existing education agreement.
• Clinical nursing experience in an inpatient hospital setting.
• Understanding of disease management, treatment, length of stay, and barriers to delivery of care.

Responsibilities

• Assessment: Identify patients using screening criteria and conduct comprehensive evaluations.
• Care Planning: Develop, adjust, and coordinate care plans to address medical and social needs.
• Patient Support: Guide self-management using motivational interviewing and coaching.
• Education & Advocacy: Educate healthcare teams and patients; advocate for patient rights and self-care.
• Collaboration: Work with multidisciplinary teams to ensure timely, high-quality care.
• Process Improvement: Lead initiatives to enhance care transitions and policy adherence.
• Advanced Care Planning: Provide Goals of Care education for informed decision-making.
• Data Analysis & Compliance: Track metrics, analyze trends, ensure regulatory compliance.
