Job summary

Intermountain Health St Vincent Regional Hospital in Billings, Montana seeks an RN Hospital Care Manager I to deliver comprehensive care management across Inpatient and Outpatient services. Responsibilities include developing patient-centered care plans, coordinating with multidisciplinary teams, and supporting patients and families through education and advocacy.

The role covers weekdays, weekends, and holidays with a 40-hour workweek.

Qualifications

• Current RN license in state of practice.
• BSN required; must obtain BSN within 4 years if hired/promo per agreement.
• Inpatient hospital nursing experience and familiarity with hospital terminology.

Responsibilities

• Assess patients using screening criteria and conduct comprehensive evaluations.
• Develop, adjust and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate and advocate for patients and healthcare teams; support self-care.
