Job summary

Providence St. Joseph Hospital Eureka in Eureka, CA is seeking an RN Care Manager for a Per-Diem role with 8-hour, variable shifts.

The RN will carry out effective case management for complex patients, coordinating their care across the continuum and communicating with hospital staff and external resources to ensure appropriate utilization and optimal outcomes. Required qualifications include an Associate's Degree in Nursing or nursing diploma and California RN license; 2 years of acute care

Qualifications

• Associate's Degree in Nursing or Nursing Diploma required at hire.
• California Registered Nurse License required at hire.
• 2 years of acute care experience in Medical-Surgical or Inpatient setting.
• 1 year of care management or utilization review experience or completion of TIPS/Case Management Orientation.

Responsibilities

• Plan, coordinate, negotiate, procure and manage care for complex patients and families across the continuum of care.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate utilization of resources.
• Monitor medical records for variances to clinical and financial standards and intervene to optimize outcomes for patient and hospital.
