Job summary

Heritage Health Network is seeking an experienced Registered Nurse Care Manager (RNCM) in Palm Springs, CA to provide clinical oversight for a team under the ECM program. The role blends remote clinical work with field-based visits to support care coordination and patient engagement.

You will review charts, assess plans, fill gaps in care, and guide non-clinical Lead Care Managers, while collaborating with hospitals and providers to optimize outcomes.

Qualifications

• Active California RN license.
• Minimum 2 years of nursing experience.
• At least 1 year in care management, case management, or leadership.
• Experience with vulnerable populations and complex needs.
• Strong clinical assessment and care planning.
• Proficiency with EMR/EHR systems.
• Excellent communication and collaboration skills.
• Willingness to perform field-based work and in-person engagement.

Responsibilities

• Provide clinical oversight to the ECM team and guide Lead Care Managers.
• Review member charts, assess care plans, and ensure quality coordination.
• Conduct field-based visits and member engagement activities.
• Coordinate referrals and transitions with hospitals and providers.
• Participate in weekly case reviews for high-risk members.
• Ensure documentation aligns with Medi-Cal ECM guidelines and regulations.
