Advocate Health is seeking a Nursing Care Manager to coordinate patient care across the multidisciplinary team and community resources. You will identify barriers, promote evidence-based practices, and develop SMART goals with patients and families to improve outcomes.

In this role you will manage a care management caseload, ensure timely documentation, and partner with care teams to deliver high-quality, cost-effective services across diverse settings.

Competenze

• Registered Nurse license required.
• Bachelor’s Degree in Nursing or related field.
• Typically requires 5 years of nursing or 1-2 years of care management experience.

• Facilitates communications among patient/family and care teams to resolve barriers to timely care.
• Collaborates with patient/family and inter-professional teams to ensure quality, efficient and cost-effective care.
• Uses evidence-based approaches to increase patient activation and self-management.
• Identifies barriers to learning and delivery of care and reports findings with plans.
• Manages CM caseload and ensures timely completion of tasks and documentation.
• Partners with at-risk patients throughout diagnosis, treatment, and follow-up.
• Coordinates patient information and transitions between facilities and physicians.
• Coordinates referrals to internal departments and external resources as needed.
• Demonstrates age-appropriate care and uses department policies and standards.
