Job summary

MemorialCare is seeking an RN Care Manager in Laguna Hills, CA for a part-time Day shift. The role supports Care Coordination/Emergency and utilizes utilization review, discharge planning, and care management practices to improve patient outcomes.

Minimum 3–5 years Acute Care experience; CA RN license; BSN preferred; BLS required; Case Management or specialty certs preferred. Sign-on bonus available.

Qualifications

• Three to five years Acute Care experience in clinical areas, with knowledge of utilization review, discharge planning activities, and regulations.
• Computer, internet, order entry and word processing experience preferred.

Responsibilities

• Utilizes the Utilization Review plan, nursing process and practice standards for case management of patient care.
• Leads coordination of patient care across the healthcare continuum toward outcomes.
• Mentors new staff and maintains patient privacy and confidentiality.
