Qualificações

• Active RN license in state of practice.
• BSN from an accredited institution; must obtain BSN within four years if hired/promoted.
• Experience in inpatient hospital setting with disease management understanding.

Responsabilidades

• Identifies patients using screening criteria and conducts comprehensive evaluations.
• Develops and coordinates care plans addressing medical and social needs.
• Guides self-management using motivational interviewing and coaching for patients.
• Provides education to healthcare teams and patients; advocates for patient rights and self-care.
• Collaborates with multidisciplinary teams to ensure timely, high-quality care.
• Leads initiatives to improve transitions of care and adherence to policies.
• Provides Goals of Care education for informed decision-making.
• Tracks key metrics and analyzes trends for regulatory compliance.
