Qualifications

• Understanding of disease management, treatment, length of stay, and barriers to delivery of care.
• Basic computer skills including Word and Excel.

Responsibilities

• Identify patients using screening criteria and conduct comprehensive assessments.
• Develop, adjust, and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate healthcare teams and patients, advocate for patient rights and support self-care.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead process-improvement initiatives for care transitions and policy adherence.
• Provide Goals of Care education for informed decision-making.
• Track key metrics, analyze trends, and ensure regulatory compliance.
