Job summary

Intermountain Health St Vincent Regional Hospital in Billings, Montana is seeking a part-time RN Hospital Care Manager I to deliver comprehensive care management, develop patient-centered care plans, and collaborate with multidisciplinary teams across inpatient and outpatient settings.

Responsibilities include assessing patients, coordinating care plans, guiding self-management, providing education, and driving care transitions while ensuring policy adherence and data accuracy to support

Qualifications

Current RN license in state of practice.
BSN required; BSN must be obtained within four years of hire/promotion.
Hospital inpatient nursing experience and familiarity with patient terminology.
Understanding of disease management and care delivery barriers.

Responsibilities

Assessment: identify patients via screening and conduct evaluations.
Care Planning: develop and coordinate care plans addressing medical and social needs.
Patient Support: guide self-management using motivational interviewing.
Education & Advocacy: educate teams and patients; advocate for patient rights.
Collaboration: work with multidisciplinary teams to ensure timely care.
Process Improvement: lead care transitions and policy adherence initiatives.
Advanced Care Planning: provide goals of care education for informed decisions.
Data Analysis & Compliance: track metrics and ensure regulatory compliance.
