Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; must obtain BSN within four years of hire/promotion.
• Inpatient hospital-based nursing experience; knowledge of hospital terminology and processes.
• Understanding of disease management and barriers to delivery of care.

Responsibilities

• Identifies patients using screening criteria and conducts comprehensive evaluations.
• Develops, adjusts, and coordinates care plans addressing medical and social needs.
• Guides self-management using motivational interviewing and coaching.
• Educates care teams and patients; advocates for patient rights and self-care.
• Collaborates with multidisciplinary teams to ensure timely, high‑quality care.
• Leads process improvements to enhance care transitions and policy adherence.
• Provides Goals of Care education for informed decision-making.
• Tracks key metrics, analyzes trends, and ensures regulatory compliance.
