Intermountain Healthcare Inc. in Utah is seeking an RN Hospital Care Manager I to deliver comprehensive care management across inpatient and outpatient settings.

You will assess patients, develop care plans, support self-management, educate care teams, and coordinate with providers and payers to ensure timely, high-quality care. This PRN role requires BSN within four years of hire, inpatient nursing experience, and strong computer skills.

Qualifications

• Current RN license in state of practice.
• BSN from accredited institution (degree verification required).
• RNs must obtain BSN within four years of hire/promotion if no existing agreement.
• Demonstrated inpatient hospital nursing experience.
• Understanding of disease management and barriers to care.
• Basic computer skills (word processing).
• Intermediate skills in Word and Excel.

Responsibilities

• Assessment: identify patients using screening criteria and conduct evaluations.
• Care planning: develop, adjust, and coordinate care plans addressing medical and social needs.
• Patient support: guide self-management using motivational interviewing and coaching.
• Education & advocacy: educate teams and patients, advocate for patient rights, support self-care.
• Collaboration: work with multidisciplinary teams for timely, high-quality care.
• Process improvement: lead initiatives to enhance care transitions and policy adherence.
• Advanced care planning: provide goals of care education for informed decisions.
• Data analysis & compliance: track metrics, analyze trends, ensure regulatory compliance.
