Job summary

Molina Healthcare is seeking a skilled care manager RN to support care coordination activities across the care continuum. You will work with a multidisciplinary team to ensure member progress toward outcomes and to implement effective care plans.

The role involves engaging members through calls and visits, evaluating plan effectiveness, and coordinating services to optimize quality and cost-effective care. Travel may be required based on contract needs.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent combination of relevant education and experience.
• RN license active and unrestricted in state of practice.
• Valid driver license, reliable transportation, and adequate auto insurance for job travel requirements.
• Understanding of EMR and HIPAA.
• Demonstrated knowledge of community resources.
• Ability to operate proactively and detail-oriented work.
• Ability to work independently with minimal supervision and self-motivation.
• Strong written and verbal communication skills.
• Proficiency in Microsoft Office and online portals/databases.

Responsibilities

• Completes comprehensive assessments of members and determines eligibility for care management.
• Develops and implements care coordination plan with member and care team.
• Conducts telephone, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload and outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings.
• Addresses barriers to care and provides coordination assistance.
• May provide consultation to peers as needed.
• Travel up to 25–40% based on state/contractual requirements.
