Job summary

Molina Healthcare is seeking a registered nurse to support care management activities and collaborate with a multidisciplinary team to coordinate comprehensive member care across the continuum.

The role emphasizes assessment, care coordination planning, and ongoing monitoring with regular member outreach. Travel to member sites or facilities (25-40%) is expected based on state contracts and needs.

Qualifications

• 2+ years of experience in health care, preferably care management or related field.
• Active RN license in the practicing state.
• Valid driver's license and reliable transportation for travel requirements.
• Knowledge of EMR and HIPAA regulations.
• Experience working with diverse populations and coordinating with multidisciplinary teams.

Responsibilities

• Completes comprehensive member assessments within regulated timelines and determines eligibility for care management.
• Develops and implements care coordination plans with members, caregivers, physicians and others.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions and adjusts goals.
• Maintains a caseload for regular outreach and management of member needs.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates ICT meetings and collaborates with the care team.
• Provides education and motivation to support member changes.
• Identifies barriers to care and assists members to address concerns.
• May provide consultation and resources to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
